Medical Records

Health Information Services facilitates the storage, retrieval and maintenance of patient medical records; advice and education regarding clinical coding and casemix; dictation and transcription services; training and support in clinical systems; and requests regarding the release of patient information (Freedom of Information requests).

Health Information Services includes three main areas:

  1. Health Information Services (HIS)
  2. Clinical Systems Support Team (CSST)
  3. Freedom of Information (FOI)

Health Information Services

Health Information Services (HIS) provides a range of services, including:

  • the storage, retrieval and maintenance of patient medical records
  • the availability of patient medical records at point of care
  • providing statistical information to internal and external departments and clinical staff
  • providing advice and education regarding clinical coding, casemix and documentation
  • providing transcription services for outpatient letters and operation reports

Clinical Systems Support Team

The Clinical Systems Support Team (CSST) offer training, procedural documentation and workflow support across both sites (Mercy Hospital for Women and Werribee Mercy Hospital) for the following clinical systems:

  • i.Patient Manager (I.PM)
  • Clinical Patient Folder (CPF)
  • Lanier Voice M*Modal Dictation System

Freedom of Information

Freedom of Information provides advice on information for the release of patient information for health services in accordance with the Health Records Act 2001 and the Health Services Act 1988, as well as requests for information under the Freedom of Information Act 1982.

To facilitate a request, please provide as many details about your request and provide the patient’s Consent for Release of Information.

Requests for information by General Practitioners and Specialists

General Practitioners and other specialist clinicians requiring copies of medical record information to facilitate the care of mutual patients can submit their request in writing either on their practice letterhead, or with signed patient consent where appropriate.

If sending through a request for information on practice letterhead; please include patient’s full name and date of birth, and clearly state the information sought.
