My Health Record

Mercy Health is connecting to the national My Health Record system.

What is My Health Record?

My Health Record is a secure online summary of a person’s health information. My Health Record enables patients to manage their health information securely, in one place. It also allows health providers to access timely information about their patients at the point of care.

Mercy Health is participating in the My Health Record system

In early 2020, Mercy Health connected to the national My Health Record system to enable our clinicians to upload and view their patients’ health information at the point of care.

Mercy Health will upload inpatient electronic discharge summaries, pathology and radiology imaging results to a patient’s My Health Record (where one exists).

From 15 November 2023, Mercy Health will also upload specialist clinic letters to a patient’s My Health Record.

Clinicians use electronic discharge summaries to share information with your primary care provider, and others involved in your care once you leave hospital. The forms contain information such as the reason you came to hospital, whether you have any other conditions, any allergies and what medication you are taking. If you would like to see an example of an electronic discharge form, please ask your treating staff member.

You can choose who has access to your My Health Record. You can remove or control who can see your uploaded patient information by logging into your My Health Record and changing access controls. You can also choose to have a record or to permanently delete it, at any time in your life.

You can also ask Mercy Health not to upload some documents. If you have not changed the access controls in your My Health Record you will need to do this each time you are provided care.
