How to refer to Mercy Health
We accept GP and specialist referrals for all Specialist Clinics. Referrals are triaged based on clinical need.
HealthPathways Melbourne provides clinicians with a single website to access clinical and referral pathways, and resources. Pathways are created by local GPs, working in partnership with specialists, nurses, allied health and other health professionals.
Mercy Health recommends clinicians to access HealthPathways Melbourne for guidance in assessing, managing and referring for patient conditions (login required).
Referrals to outpatient clinics at Mercy Health will be triaged against clinical information and investigations based on these guidelines.
Referrals that do not meet the required clinical information and investigations will not be accepted.
HealthPathways Melbourne is available for free to health professionals working in the North Western Melbourne and Eastern Melbourne Primary Health Network catchments.
Email HealthPathways to request access or if you need to confirm your username and password.
File importing instructions
Medical Director
- DO NOT OPEN IN WORD.
- Right click on the link and then select ‘save as’ or ‘save target as’ and then save the file to a location where you can later retrieve it (e.g. Desktop or My documents).
- Open ‘Medical Director’ and then ‘LetterWriter’.
- Select ‘file menu’ >’modify template’ > ‘blank template’.
- Select ‘file’ >’import’ and then select downloaded file.
Best practice
- DO NOT OPEN IN WORD.
- Right click on the link and then select ‘save as’ or ‘save target as’ and then save the file to a location where you can later retrieve it (e.g. Desktop or My documents.
- Open a new letter and select from the ‘templates’ menu ‘import templates’. Locate downloaded file and open.
- From ‘file’ menu select ‘save as’, type in a template name and click on save.
ZedMed
- DO NOT OPEN IN WORD.
- Right click on the link and then select ‘save as’ or ‘save target as’ and then save the file to a location where you can later retrieve it (e.g. Desktop or My documents).
- Select ‘tools’ > ‘clinical WP setup’ > ‘WP templates’.
- Select ‘file’ > ‘import template’.
- Go back to the original file location, highlight the file and then click ‘open’.
- Enter the template name into the Description field. Click on OK.
Referral forms & templates
Please delete old versions of the Mercy Health referral forms from your software and desktop and discard any old paper versions and replace them with the relevant forms below.
