How to refer to Mercy Health

We accept GP and specialist referrals for all Specialist Clinics. Referrals are triaged based on clinical need.

    Referrals to outpatient clinics at Mercy Health will be triaged against clinical information and investigations based on these guidelines.

    Referrals that do not meet the required clinical information and investigations will not be accepted.

Standard information required on all referrals

  • Reason for referral
  • Relevant investigations
  • Past medical history
  • Relevant social history
  • Medications
  • Patient contact details, including need for an interpreter
  • Allergies

To refer a patient, please complete a referral and fax to the relevant area, service or specialist clinic.

File importing instructions

Medical Director

DO NOT OPEN IN WORD

  1. Right click on the link and then select Save As… (Or Save Target As…) and then save the file to a location from where you can later retrieve it i.e.​ desktop or My documents.
  2. Open Medical Director and then LetterWriter
  3. File menu > Select modify template > Select blank template
  4. File > Select import – locate downloaded file

Best Practice

  1. Right click on the link and then select Save As… (Or Save Target As…) and then save the file to a location from where you can later retrieve it i.e. desktop or My documents. ​Open a new letter and from templates menu select Import templates
  2. Locate downloaded file and open
  3. From File menu select Save as. Type in an appropriate template​ name and click on save

  2. Click on Tools > Clinical WP Setup > WP Templates
  3. File > Import Template
  4. Find the file where you saved it and highlight it and then click Open
  5. Enter the template name into the Description field. Click on OK

Referral forms & templates

Please delete old versions of the Mercy Health referral forms from your software and desktop and discard any old paper versions and replace them with the relevant forms below.
