A referral from a health professional or a community worker is needed for all families wishing to access early parenting services.

- Indigenous status
- Referring details
- Health insurance details
- Preferred language and interpreter requirements
- Medicare eligibility & number
Clinical information
Please provide the following key clinical details:
- Relevant medical history
- Relevant social history or special needs
- Current medications
- Allergies or warnings.

Referral process
Please fill out all of the information on the referral form, then print and fax to 03 8416 7650.
Step 1
Health professionals
Complete the referral form located on the website. Please complete the form neatly and provide as much detail as possible.
An initial assessment is conducted based on the information provided in the referral. Clients are triaged for an intake phone call based on the outcome of this assessment.
Step 2
Families
The information included in your referral is checked by an intake team member. You will be contacted by the intake team within four weeks.
