Social workers can help you to access services and entitlements, and assist with all aspects of discharge planning.
Current inpatients and outpatients of the hospital may be referred to see a social worker by hospital staff, or you may refer yourself by simply contacting the social work department.
Social workers can provide:
- counselling and support
- advocacy within the hospital and community
- information about community resources
- referral to other services
- preparation for discharge from hospital.
