At Mercy Health, the Primary Care Liaison team helps GPs to navigate services at our two hospitals: Mercy Hospital for Women and Werribee Mercy Hospital. The team works to improve communication between the hospitals and primary care.

 

The Primary Care Liaison Unit promotes an integrated, person-centred healthcare system by improving access to services, promoting continuity of care across health services and building strong working relationships that:

  • promote sustainable collaboration and partnerships between services
  • improve the quality of health care
  • optimise resource use and the efficiency of care across multiple providers

We now accept GP and specialist referrals for all Specialist Clinics via Healthlink. For assistance please contact primarycare@mercy.com.au.

eReferrals via HealthLink

Mercy Health is now receiving digital referrals for specialist clinics in our hospitals.

eReferral will allow GPs to submit pre-populated digital referrals directly to our specialist clinics at Mercy Hospital for Women and Werribee Mercy Hospital. It is free to use and integrated into GP practice management software via HealthLink. eReferral allows GPs to efficiently and effectively send referrals to our specialist clinics, and allows for attachments such as pathology and radiology results, clinical images and appropriate PDF documents to be securely uploaded.

Statewide Referral Criteria

The statewide referral criteria provide guidance to referring clinicians about which patients will be seen in adult public specialist clinics and what investigations are required to support triage.

The Primary Care Liaison Unit is leading this important piece of work for Mercy Health as part of the Victorian Government Specialist Clinics Reform, so please contact us if you have any queries specific to Mercy Health specialist clinics.

We facilitate the key contact points to obtain clinical advice, action feedback, assist with navigation of services and clinical handover coordination concerns for primary care providers.

Shared maternity care affiliates

    HealthPathways Melbourne provides clinicians with a single website to access clinical and referral pathways, and resources. Pathways are created by local GPs, working in partnership with specialists, nurses, allied health and other health professionals.

    Mercy Health recommends clinicians to access HealthPathways Melbourne for guidance in assessing, managing and referring for patient conditions (login required).

    Referrals to outpatient clinics at Mercy Health will be triaged against clinical information and investigations based on these guidelines.

    Referrals that do not meet the required clinical information and investigations will not be accepted.

    HealthPathways Melbourne is available for free to health professionals working in the North Western Melbourne and Eastern Melbourne Primary Health Network catchments.
